Step 3. Ship Samples:

Ship only AFTER you have faxed or scanned your registration forms to us. You may also include copies in the box, but only to confirm your registration.

Step 4. Verify information:

Once we receive your entry forms we will send you a receipt with your product information. Please review, make any corrections, and fax back to 800-238-7972 or email to tsulinski@tastings.com.

45 days after the deadline your results will be mailed and emailed to your contact information provided on the registration form. The same information will be posted next to your review on Tastings.com, unless otherwise noted. Results will only be released upon full payment of the registration fees.

International Customers: Submit Your Samples in Five (5) Simple Steps

(Do not ship samples before reading all of these steps. Please do not simply ship via post without completing these steps, your samples will probably not arrive.)

Step 1. Download and Review Schedule and Entry Form:

Step 2. Submit Registration Form(s):

Please complete one form per entry. Be sure to print clearly and fax completed form to 312.226.7858 or scan them to tsulinski@tastings.com.

Step 3. FDA Registration:

If you already have an FDA registration number skip Step 3 (this step) and go to Step 4-Prior Notice. If you do not have an FDA registration number, you MUST apply for one. Follow this link below to apply:

Step 4. Prior Notice:

In order to ship into the US you must have a Prior Notice. Click on the link below for more details. You will need to supply this number to your courier or shipper.

Step 5. COLA Waiver (Certificate of Label Approval):

(If you already have COLA approval, send your COLA approval document with the shipment. If you do not have a COLA approval, follow the directions below.)

Once we’ve received your entry form WE will apply for your COLA waiver. Normally this process has a quick turn around time–usually within 3 days. We do not need to wait for you to apply for your FDA number to apply for your COLA waiver. Once we receive your COLA waiver we will send you a copy to include with your shipment and you’re ready to ship your samples. Use the link below to begin the COLA waiver process.

We need the following information about your sample shipment in order to process your COLA waiver:

[TOTAL NUMBER OF BOTTLES/ QUANTITY OF PRODUCT, PRODUCT NAME/TYPE (Wine, Beer, Spirits, RTD, Mixer)]

NUMBER of Bottles of Each Product [PRODUCT NAME & PROOF – LIST INDIVIDUALLY WITH ABV (Alcohol by Volume)]

Additionally, each individual sample bottle or container must bear a label stating “Sample Only – Not for Sale” or similar phrase. If it is a wine, the product sample must also contain a “Contains Sulfites” label.
