If you wish to submit a commercial wine, spirit, or beer for review, you can do so by downloading PDF versions of our registration forms. All the necessary information, including our tasting schedule and mailing address, is included in the downloadable forms below. All downloads are PDF files and you will need Adobe Acrobat Reader installed to view and print these forms. If you do not have Acrobat Reader installed you can download this free software from Adobe.

(The forms should automatically download to your harddrive after clicking the link, even if you do not have Acrobat installed. If clicking the link appears not to do anything, check your Download folder, Desktop, or other folder that your browser has been configured to download to; the form has probably been downloaded there.)
